The clinical forms that we provide have been used in over 100 agencies since 1997. Can I make changes to the forms?

The forms come in Microsoft^© Word^© for purposes of printing. You may use text boxes to revise existing text, instructions are provided.

What kind of printer do I need?

You can use either ink jet or laser. Your printer should be capable of printing 1/4" margins. The better the output resolution, the better your forms will look.
