Our clinical forms package provides a comprehensive set of paper documents that helps reduce staff work while meeting practice standards. Version 8.0 is now available in portable document format (pdf). Forms are customized with your logo, name, address and phone numbers. Forms may be edited with pdf software such as Adobe Acrobat (not Acrobat Reader) or LightningPDF Professional, available from the software maker at additional cost. Our forms are not an electronic medical record. Releases of Information have been updated for HIPAA if needed. Patient portion of Intake & Assessment, Releases of Information and HIPAA Privacy Notice are available in Spanish.Forms are $150.00 plus tax, delivered to your email address.
